Community Choice Nursing Facility Transition Project
Providing Supports for Persons Transitioning from Nursing Homes
The goal of this program is to provide individuals, regardless of their disability and age, an opportunity to choose community based living as a viable alternative to nursing home residency.
This program is open to residents of a nursing home who are receiving Medicaid and desire to live in the community.
We will assist people with returning to their own home, moving in with family members/friends or finding new housing.
What is the Process?
A referral can be made by the individual residing in the nursing home, family or friends, a social worker or other professional. Persons interested in this program should contact Mona Khaled, Community Choice Program Coordinator, at (269) 345-1516 or (800) 394-7450 or mkhaled@drccil.org
Phone Screening
The initial interview will consist of a more detailed explanation of the Community Choice program. Information will be obtained to determine what community supports and programs will best meet the needs of the individual considering moving from a nursing home.
Assessment
An initial meeting will be set up with the individual, and any other support persons they desire to be present.
Plan Development
A person centered plan will be developed. It is important that the individual desiring to move from the nursing home be actively involved in the planning process.
Housing
Individuals who don’t have a home to return to, will be provided with information on accessible, affordable housing.
Home Assessment. Home assessments are often done to determine if home modifications and assistive devices will be of benefit to the individual.
Support Services
Individuals will be provided assistance in arranging the necessary support services to maintain community living. Examples of support services include in-home personal and medical care, housekeeping, transportation, and health care.
The transition process can take several months to complete and in some cases barriers can cause the process to be longer.
The individual may terminate the transition process at anytime if he/she chooses to remain living in the nursing home.
